- Maintain patient/client clinical records with confidentiality and safety
- Efficiently complete assigned admissions
- Communicate effectively with physicians, referral sources, patients, families, and the hospice interdisciplinary team
- Document all clinical assessments, identified needs, and pertinent information accurately
- Adhere to all policies and procedures
- Ensure compliance through accurate and timely documentation in the organization's electronic health record
- Perform any other duties as assigned
- Must be a licensed registered nurse in the state of operation
- Minimum of 2 years experience in hospice, home health, or other health care service delivery setting
- Current CPR certification
- Valid Driver's License, vehicle insurance, access to a reliable vehicle, or public transportation
- In LA: a minimum of two years full-time experience as an RN, or two years of full-time clinical experience in hospice care as a licensed practical nurse
