    Your Role as a Home Health Physical Therapist:

    • Deliver home-based physical therapy to patients recovering from illness or injury, helping them regain mobility and functionality.
    • Collaborate with a dedicated home health team, including RNs, LPNs, CNAs, HHAs, OTs, and SLPs, to focus on patient care plans and goals.
    • Conduct assessments, observe movements, document progress, and adjust care plans as necessary.
    • Guide patients through exercises and hands-on therapy to alleviate pain and enhance mobility.
    • Educate patients and their families on care plans, goals, exercises, and self-care techniques.

    What We're Looking For:

    • Graduates of an accredited Physical Therapist Program with an active PT license in Michigan (NEW GRADS WELCOME).
    • At least 1 year of clinical experience, preferably in home health within the last 5 years.
    • Current CPR certification.
    • Strong clinical judgment, excellent interpersonal skills, dexterity, and a compassionate approach.
