    Job Summary

    Revival Home Health is seeking a Licensed Practical Nurse (LPN) to provide routine care and general observation of clients' health status in a home healthcare setting. The LPN will work under the supervision of a Registered Nurse and be responsible for communicating with clients and their families/guardians.


    Key Responsibilities

    • Provide direct care to patients in accordance with a comprehensive care plan.
    • Document patient services accurately and timely.
    • Assess and educate patients and their caregivers on self-care and treatment plans.
    • Participate in coordination of home care services and ensure compliance with state and federal regulations.

    Requirements

    • Graduate of an accredited practical or vocational nursing program with current licensure to practice in Maryland.
    • At least one year of nursing experience, preferably three years or more.
    • Experience in community health, hospice, or med/surg settings preferred.
    • Commitment to clinical and documentation excellence.
