    Nurse Practitioner – PRN/Part Time

    Home Risk Assessments

    About the Position:

    Matrix providers conduct thorough health assessments during visits, providing crucial insight into a member's overall well-being that may be missed during regular office visits. These assessments help enhance care quality and close multiple care gaps in a single visit.

    Responsibilities

    Matrix Provider – What to Expect:

    • Perform adult/geriatric assessments, diagnosis, treatment, health education, referrals, and clear documentation according to Matrix guidelines
    • Collaborate with healthcare professionals and caregivers
    • Work in various settings including homes, facilities, and telehealth

    Nurse Practitioner - PRN/Part Time - Home Risk Assessments

    Type: PRN/Part Time - Flexible-Nurse Practitioner

    PAY: $90.00 or $95.00 (based on location) per completed assessment

    Location: Home, SNF, Telehealth, and other community settings in Blanchester, Ohio

    Hours: Day, Evening, Night, Weekend shifts with flexible scheduling

    Benefits: PRN/Part Time providers receive 401(k) and limited Voluntary Benefits

    Mileage reimbursement available

    Qualifications

    Must Haves:

    • Master's Degree or equivalent experience with NP licensure
    • Current RN and NP licensure with prescription authority
    • Board certified in a Matrix-approved specialty
    • Minimum of 1 year as a Nurse Practitioner
    • Strong computer skills and communication abilities
    • Flexible and adaptable to change

    Travel Requirements:

    • Valid driver's license, vehicle, and insurance
    • Travel to meet patients as needed

    Primary Location: Blanchester, OH Job: Clinical - Nurse Practitioner or NP. Regular Shift. Full or Part-Time. Day Job, Travel. No on-call, evening, or weekend hours unless desired. Work from home opportunity.
