Job Summary
The Health Navigator position provides services that include connecting HOPWA Program to tenants' care team. This position provides case management and care coordination, health education, interdisciplinary collaboration and coordination.Responsibilities
- Conducts assessments which should highlight the Tenant strengths, limitations, risk factors and internal/external support and service needs to optimize the tenants' ability to access and maintain health care services.
- Act as a liaison between the HOPWA program, community medical clinic and other healthcare providers, coordinating care for a population of tenants with similar complex needs who require assistance accessing health care services or adhering to health care plans.
