About the Position:

During a visit that can last up to one hour, Matrix providers assess a member's current health, medical history, medication adherence, social environment, and other risks. This provides valuable insight into a member's overall health and well-being that can be challenging to capture during routine office visits. The Matrix Comprehensive Health Assessment helps enhance the quality of care and allows us to potentially address multiple care gaps in one visit.

Responsibilities

Matrix Provider – What to Expect:

• Conduct Adult/Geriatric Assessments, medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation following Matrix guidelines and protocols
• Work closely with physicians, case managers, social workers, family members, key caregivers, and appropriate ancillary medical personnel
• Collaborate with Primary Care Physician (PCP) on patient education and provide follow-up care
• Deliver services in various settings including Home Visits, Skilled Facility Visits, and Telehealth appointments

Nurse Practitioner - PRN/Part Time - Home Risk Assessments

Type: PRN/Part Time - Flexible-Nurse Practitioner

PAY: $90.00 or $100.00 based on the county per completed Assessment

Location: Home, SNF, Telehealth, and other community environments in Nahunta, Georgia

Hours: Day, Evening, Night, weekend, Flexible Scheduling

Benefits: PRN/Part Time providers receive non-benefits except for 401(k) and limited Voluntary Benefits

Mileage Reimbursement plan

Qualifications

Must Haves:

• Master's Degree required OR equivalent experience and successful completion of NP licensure
• Current RN and NP licensure in the practice state including prescription authority or eligibility for prescriptive authority
• Board certified by the AANP, ANCC, or the AACCN in a Matrix-approved specialty
• Hold a current BLS, ACLS, or CPR certification
• Minimum of 1-year experience as a Nurse Practitioner
• Strong computer skills and familiarity with health/medical record software
• Excellent verbal and written communication skills with patients, clients, and colleagues
• Adaptability and comfort with frequent changes

Travel Requirements:

• Ability to travel, possess a valid state driver's license, drive a car, and provide proof of adequate automobile insurance
• Travel necessary to reach patients at their residences
