    Job Description:

    SUMMARY OF POSITION

    CCBHC Linkage and Engagement Provider provides linkage services which would include resources to clients, families, community partners and referral sources. Provides emergency mental health consultations. Provides follow-up appointments when it is clinically appropriate. Expected to actively participate in a variety of cross-functional teams and performance improvement initiatives.

    ESSENTIAL DUTIES & RESPONSIBILITIES
    • Provides linkage services to clients, families, community partners and referral sources. This includes providing contacts for resources, outreaching resources with clients, setting up resources for clients, and linking clients to Centerstone services.
    • Required to complete all required documentation, comply with contracts, grants, standards, regulations, and laws associated with this position in addition to working under the direction of the Assessment Clinical Coordinator when performing job tasks and providing services. Attend staff meetings, clinical staffing's, and other internal meetings as assigned. Maintain appropriate communication with other employees and work as an effective team member.
    • Provide follow-up appointments when it is clinically appropriate to the service recipient and provide Brief Therapy until an appropriate referral has been completed for on-going treatment or no further services are indicated. Complete IATP if that is needed for clients to start services at Centerstone.
    • Provide emergency mental health consultations at office locations in an effort to resolve mental health crisis regarding residents in our service area.
    • Participate in training designed to enhance job-related skills.
    • Provide community education as assigned.
    KNOWLEDGE, SKILLS & ABILITIES
    • Effectively communicate via written, verbal, in person and virtual methods.
    • Skill in customer service techniques.
    • Skill in clinical assessment of mental health and substance use diagnoses.
    • Skill in developing problem solving strategies.
    QUALIFICATIONS

    Education Level

    Bachelor's Degree in Human Services Field, or Bachelor's Degree with MHP Status.

    Years of Experience

    Experience working with the public or a demonstrated interest to work with different population.

    Certification/Licensure
    • Position requires valid driver's license and safe driving record.
    • CPR certification within 90 days of hire.
