Job summary
The LPN provides support to Gateway's high acuity adult programs under the umbrella of Gateway's nursing department and under supervision of program RN.The position has two primary functions: promote the overall psychiatric and physical health of clients through screenings and coordination of care with primary and specialty care providers, monitoring targeted health indicators;and provide support to RNs and other team members in monitoring caseloads and required documentation, communicating with clientsand collateral contacts on appointments,medications,laboratorywork,and referrals.
