- Effectively complete OASIS documentation in accordance with agency policies and state and federal regulations, demonstrating knowledge of OASIS documentation requirements.
- Utilize the nursing process to assess patient/caregiver needs to provide quality care in accordance with evidencebased practices and regulations.
- Develop and communicate realistic plans of care for each patient, including patient/caregiver involvement in the process.
- Serve as Case Manager for assigned patients, managing caseloads and visit times according to agency productivity standards.
- Implement medical plans of care and nursing interventions necessary for safe and effective quality patient care.
- Evaluate the implemented plan of care, assessing identified problems, care provided, and patients' responses to ensure quality care delivery.
- Follow agency protocol with McKesson processes, including lockingdown visits and documentation completion.
- Perform patient and caregiver education to ensure informed decisionmaking.
