    KEY RESPONSIBILITIES:

    • Conduct thorough assessments and evaluations of members' physical abilities, functional status, and mobility limitations.
    • Develop individualized treatment plans based on members' needs, goals, and medical history, utilizing evidence-based practices and therapeutic modalities.
    • Administer a variety of therapeutic interventions and exercises to improve strength, flexibility, balance, coordination, and mobility.
    • Educate members and their families about their condition, treatment options, and preventive measures to promote self-management and long-term wellness.
    • Monitor members' progress throughout the course of treatment, adjust treatment plans as needed, and document outcomes accurately and comprehensively.
    • Collaborate with interdisciplinary healthcare team members to ensure coordinated and comprehensive care delivery.
    • Communicate effectively with members, families, and caregivers to provide support, address concerns, and promote adherence to treatment plans.
    • Adhere to all regulatory requirements and documentation standards, including those set forth by Medicaid and Medicare, to ensure compliance and reimbursement for services rendered.
    • Participate in quality improvement initiatives and performance improvement activities to enhance the delivery of physical therapy services and achieve optimal member outcomes.
    • Stay up to date on current developments in physical therapy practice, research, and technology through continuing education and professional development activities.
    • Other duties may be assigned.

    EXPECTATIONS:

    • Successful completion of background check and drug screen.
    • Maintain compliance with all Gloria Gates CARE standards, including Conduct, Compliance, and all other policies and procedures.

    EDUCATION AND EXPERIENCE:

    • Doctor of Physical Therapy degree from an accredited school.
    • State licensure as a Physical Therapist.
    • Certification by the American Physical Therapy Association (APTA) preferred, but not required.
    • 2 plus years of clinical experience with developed competence in assessment, treatment planning, and therapeutic interventions, as well as familiarity with documentation requirements and interdisciplinary collaboration.
    • Experience working with Medicaid and Medicare populations preferred.
