    Works as a safety net, and coordinator of care for our heart failure patients who may be discharging from the hospital in the next several days.

    Provides education, acts as a liaison with the patient, physician, family, other health care personnel, hospital administration, agencies and community groups throughout the patient's hospital stay or service.

    Supports and guides patient and family in the health care decision-making process including but not limited to admissions, diagnostics, surgical/medical interventions, discharge, and support services.

    Serves as point of contact, clinical leader and resource to staff and administration. Provides patients and family with needed information for the transition from active treatment to managing their condition independently. Patient care may involve inpatient and outpatient services.


    Qualifications:


    This navigation role will work in close cooperation with unit nursing and leadership, our transitions of care teams, physicians, advanced practitioners, and requires excellent communication skills, attention to detail, and the ability to adapt to changing environments.

    The Inpatient CHF RN Navigator will serve to assist in development and improvements to the CHF program, cooperating with all stakeholders with a primary focus on quality.

    Therefore, this nurse will be both a leader as well as someone who can effectively execute the day-to-day priorities of the role.

    A Bachelors in Nursing (BSN) from an accredited School of Nursing (or must obtain BSN within 2 years of hire)


    Required:
    Three years related work experience, with at least one of those years being on an inpatient, hospital unit


    Preferred:
    CHF Experience of at least 1 year

    Patient Satisfaction and Navigation Experience of at least 1 year

    Former experience in leadership, quality projects or education
