    POSITION SUMMARY

    Under the supervision of the CMO with clinical oversight by the Medical Director, provides medical care to participants which includes assessment and development of a plan of care in collaboration with the interdisciplinary team. Provides direct patient medical care and evaluation of the effectiveness of the plan of care. The physician functions in a collegial relationship with the employees to promote optimal, safe, effective, and efficient patient-centered care. Makes independent decisions regarding medical needs and health regimens and carries out medical procedures as indicated. The quality of the medical care is under the direction of the Medical Director.

    ESSENTIAL FUNCTIONS
    Provides initial and semi-annual participant medical assessments, care plan development and implementation of care plan.
    Evaluates the effectiveness of the participant's plan of care and revises as appropriate.
    Obtains a complete health history and records the findings in a systematic organized manner.
    Performs physical examinations, orders and evaluates appropriate laboratory and diagnostic tests in a cost-effective manner and records systematically.
    Performs functional status evaluations and/or mental status examinations using standardized procedures.
    Triages status changes in participants and facilitates appropriate management of identified problems.
    Provides assistance and oversight to PACE medical staff in the scope of their duties as needed.

    TREATMENT
    Provides preventive health care and health promotion for participants.
    Treats and manages acute chronic illnesses in the health center, nursing home, group home, or participants' homes.
    Regulates and adjusts medications as needed and in a cost-effective manner.
    Facilitates medical specialist consultation as needed and oversees participant's use of medical specialists and inpatient care.
    Educates participants and families regarding health maintenance and chronic medical conditions.
    Coordinates participant care when participants are hospitalized and communicates participant status to the interdisciplinary team.
    Specific nurse practitioner supervision and collaboration requirements are as follows:
    ?To be available for consultation
    ?To make recommendations for IDT concerns
    ?To be available 24 hours day/seven days per week in person or by phone regarding any questions or concerns regarding patient care
    ?To meet regularly to review all patients that the providers have assessed and documented on in the week prior and discuss any outstanding issues, concerns, new diagnoses and the specificity of those diagnoses.
    Interprets diagnostic tests in the scope of current practice in accordance to patient needs and required care
    Performs in-office medical procedures in the scope of current practice in accordance to patient needs and required care

    OTHER
    Provides in-service training as needed.
    Interprets program to interested individuals and groups.
    Functions as a member of the interdisciplinary team and participates in the development of the comprehensive plan of care and ongoing monitoring of the participants health status, providing medical knowledge when needed.
    Maintains current, accurate documentation of health care services provided, coordinated or contracted.
    Serves on various committees of the organization as requested.
    Prepares and submits reports, including services provided, as required.
    Attends pertinent health care conferences and courses to maintain knowledge of current trends in health care.
    Protects privacy and maintains confidentiality of employees, participants and sensitive agency information.
    Advises the Medical Director and CEO on medical and health matters.
    Handles potentially infectious specimens with appropriate biohazard precautions, and practices universal precautions.
    Performs related duties as assigned.
