    Compensation:
    $36 to $41 per hour


    Nexus HR is looking for a Licensed Vocational Nurse for a reputable, DHCS-accredited addiction treatment facility in San Jose, CA.


    The ideal candidate should have at least one year of experience working with chemical dependency, mental health, and/or co-occurring disorders.

    About the Job


    The licensed vocational nurse is responsible for providing direct client assessment, medication oversight, and medical case management services to beneficiaries in a residential substance use treatment setting under the auspices of the medical director.

    He/she works collaboratively with the treatment team to provide education and support to beneficiaries regarding their medications and to coordinate referrals to outside medical services.


    Duties and Responsibilities:
    Complete beneficiary health screening questionnaire and medication reconciliation forms during the beneficiaries' intake process. Medical data should be collected and recorded in a comprehensive, accurate, and systematic manner

    Complete pill counts, audit medication log, oversee client self-administration of medications, coordinate medication refills, and resolve beneficiary issues with prescriptions

    Participate in multidisciplinary team meetings and case conferences

    Provide medical case management services to include primary care, obstetric, mental health, and MAT appointments

    Receive medical documentation from clients, upload it to the electronic health record system, and notify the medical director for review

    Complete withdrawal management scales for clients in a detoxification program, document changes and report to the medical director as necessary

    Work with the medical director to triage beneficiaries' medical needs and daily medical status

    Oversee TB testing for clients and staff

    Perform additional duties as assigned by the supervisor and/or medical director


    Qualifications:
    Minimum 1-year experience as a licensed vocational nurse with experience treating clients with chemical dependency, mental health, and/or co-occurring disorders

    Maintains current BLS certification and active California LVN status

    Must be able to work independently under the auspices of the medical director

    Must discharge duties within the licensure and scope of a licensed vocational nurse


    Ability to effectively and efficiently perform all job duties utilizing clear written and verbal communication in a collaborative team environment.

    Exhibits excellent organizational and time management skills with the ability to prioritize workload in a fast-paced, intensive, client-centered environment

    Ability to work effectively with people differing in value systems, ethnicity, cultural backgrounds, language capabilities, and disabilities

    Utilize tact and diplomacy; exercise good judgment and respond calmly in emergencies

    Must obtain CPR/First Aid certification, annual TB test, and complete annual required training.
