    As a Physical Therapist Assistant, you will have the unique opportunity to provide exceptional care to patients in the comfort of their own homes. With Millennium Home Care, you will be at the forefront of delivering personalized and compassionate care, making a direct impact on the well-being and quality of life of our patients.

    Responsibilities:

    Provide high-quality, evidence-based physical therapy services to patients in their homes, under the guidance and supervision of a licensed Physical Therapist.

    Assist in the development and implementation of patient-specific treatment plans, focusing on promoting mobility, independence, and overall physical well-being.

    Conduct comprehensive assessments to identify patients' needs, goals, and potential barriers to recovery.

    Collaborate with interdisciplinary team members, including physicians, nurses, and other therapists, to ensure integrated and holistic patient care.

    Educate patients and their families on therapeutic exercises, adaptive techniques, and strategies to promote independence and enhance overall functioning.

    Monitor and document patient progress, adjusting treatment plans as necessary to achieve optimal outcomes.

    Continuously stay updated on the latest advancements in physical therapy techniques, ensuring the delivery of cutting-edge and evidence-based care.

    Adhere to all regulatory and compliance standards, ensuring the highest level of patient safety and confidentiality.

    Requirements:

    Current state license as a Physical Therapist Assistant in Florida.

    A Minimum of 1 year of experience in home health, or a related field

    Excellent communication and interpersonal skills

    Ability to work independently and as part of a team

    Strong problem-solving and critical thinking abilities
