Participates as a member of the interdisciplinary healthcare team to facilitate the continuum of care and education of patients with chronic diseases.#Utilizes Medical Nutrition Therapy ( MNT)and evidence-based care to provide effective and ethical nutrition counseling to patients.#Employs motivational interviewing techniques to facilitate behavior change.#Assess data reflective of patient#s nutritional requirements relative to his or her age specific needs required.#Collaborates with Population Health for nutrition articles, media programs and /or outpatient programs.

# Bachelors degree or higher and completion of an ACEND accredited Dietetic Internship. 3-5 years of experience in nutrition counseling including motivational interviewing. Commission on Dietetics Registration. NH licensure required upon hire.#
