- Provide trauma-informed, culturally aligned, and person-centered peer counseling services for individuals living with chronic behavioral health conditions
- Work collaboratively as part of a multi-disciplinary team to perform consistent outreach to clients
- Utilize community resources to provide effective linkage and follow-up care
- Assess client needs and provide support using wellness & recovery model, trauma informed principles, harm-reduction, and de-escalation techniques
- Excellent communication skills and ability to work in multi-disciplinary teams
- Utilize safety-training protocols and practice safe outreach practices
- Document in Electronic Health Record and use HIPAA-protected communication software
- Work on team de-briefing procedures and self-care protocols
- Travel with clients and maintain regular attendance
- Demonstrate resilience, emotional regulation, and self-care
- Maintain administrative and service documentation
- Establish clear and cooperative communication with staff, clients, and partner agencies
- Represent the organization at stakeholder meetings and provide feedback
- Participate in various meetings and staff development activities
- Perform assigned duties
- High School Diploma required, Associate of Arts (AA) degree preferred
- 2-3 years of direct personal experience with behavioral health system or drug treatment services
- Completion of addiction or drug counseling certificate program or equivalent education
- Knowledge of wellness and recovery model, harm reduction, crisis intervention, etc.
- Ability to travel within San Francisco
- Physically able to walk/stand and lift/carry items
- Experience in outreach to at-risk populations preferred
- Experience with mental health conditions and substance misuse
- Strong communication and organization skills
- Knowledge of service delivery documentation and HIPAA standards
- Commitment to mission of providing mental health services
- Interpersonal skills and ability to work with diverse staff
- Ability to provide services in threshold languages preferred
