- $91,000 to $154,000 annually for full time visits per week)
- $37,000-$88,000 for Part Time visits per week)
- Full Time - $ Paid after 90 days of employment. Must have worked an average of 30 hours a week in the prior 90 days.
- Part Time - $ Paid after 90 days of employment. Must have worked an average of 20 hours a week in the prior 90 days.
- Create and implement skilled care plans based on evaluations and assessment results in collaboration with physicians.
- Help pediatric and adult patients develop or regain physical, neurological, cognitive, and social/emotional functions to enhance independence.
- Must have a current Colorado Physical Therapist license in good standing.
- Preferably, at least one year of recent experience as a Physical Therapist.
- Ability to work independently and manage time effectively.
- Exceptional attention to detail and strong organizational skills.
- Excellent documentation skills with knowledge of home health agency regulations.
