    Position SummaryPractitioner is a Medical Doctor who is skilled in their ability to provide the full range of core primary care services including evaluation, diagnosis, history and physical exam, referring to specialists, medication reconciliation, and provision of care to patients presenting to our health center.

    Practitioner is willing to work in a diverse environment such as that seen at a Federally Qualified Health Center, is comfortable working with People Living With HIV (PLWH), the LGBTQ+ Community, patients struggling with opiate use disorder, and providing primary care-level gynecologic services.

    Practitioner keeps up with changing guidelines regarding general patient care and patient-specific clinical conditions. Discusses wellness and adherence with patients as necessary.

    Key Duties and Responsibilities General Clinical Practice:

    The physician will:
    Perform and document complete, system-focused, or symptom-specific history & physical examinationAssess the need for and perform additional screening and diagnostic testing, based on initial assessment findingsOrder, execute, and manage appropriate clinical tests; analyze and interpret diagnostic images; formulate differential diagnoses by priorityPrioritize accurate data collectionPerform visits on assigned patient populationDocument clinic progress notes, plans of care, and evaluations appropriately in the EHR for medical record keeping, clinical tracking, compliance, billing, and reporting purposesAdminister or prescribe appropriate pharmacologic and non-pharmacologic treatment modalities and perform medication reconciliationUtilize evidence-based, approved practice protocols in planning and implementing careInitiate appropriate referrals and consultations when services are outside of the scope of the Physician's practiceProvide specialty specific consultation services upon request and within specialty scope of the Physician's practice and according to their scope of privileges Facilitate patient transitions between and within health care settings, such as referring, transferring, and discharging patientsMaintain clinical competency, as demonstrated by providing and directing patient care, continuous quality improvement, and continuous medical education per LSBME & respective medical board requirement Clinical Practice Specific to this Position:
    The physician will:

    Maintain a patient panel (after an appropriate introductory period) with a minimum of 1700 visits annuallyWork with medical leadership and Quality Council to implement quality initiatives into primary care practiceBe willing to see (with appropriate guidance from provider peers/supervisors) patients urgently when clinically indicated and scheduled by CrescentCare's Triage Nurses.

    Complete documentation and notes within a maximum of 7 calendar days.


    Regulatory:

    The physician will:

    Maintain compliance with FTCA, LSBME, OSHA and other regulatory agencies' statutes and standards including CrescentCare Standards of Care and CrescentCare Clinical Staff GuidelinesComply with all Agency and regulatory safety standards and infection controlComply with all regulatory and disclosure paperwork required to participate in staff/provider educational programsFollow CrescentCare policies and procedures related to patient care Operate within the scope of practice as outlined by the privileges granted and this job description.

    Participate as required in Peer Review processObtain proper informed consent as a prerequisite to any procedure requiring informed consent.

    The physician will:

    Assist in educational programs for staff and participate in team meetings Provide education to patients and their support systems in areas as clinically indicated for disease management, prevention, and wellness Promote teamwork, good interpersonal relations, and communication among staff at CrescentCare Participate in Health Fairs, employee and volunteer health programs, immunization campaigns, and telehealth visits as appropriate Assume additional responsibilities as necessary QualificationsSuccessful completion of an ACGME or AOA-accredited residency in internal medicine, pediatrics, family medicine, or other specialty service provided by CrescentCare under its scope of services as a FQHC and defined on the HRSA Form 5A.

    Foreign equivalent training determined by CMO and Medical Review Committee.
