    The Hospice Chaplain provides direct care to clients and participates as a member of the Interdisciplinary Group

    MINIMUM QUALIFICATION STANDARDS:

    I. Education:

    CPE (Clinical Pastoral Education) Preferred

    II. Experience:

    Three to five years of experience in pastoral counseling or counseling / ministry

    Preference for some experience with dying clients and/or families and willingness to work with them

    III. License Requirements:

    N/A

    PERFORMANCE REQUIREMENTS:

    I. Performance Standards:

    Serves on the Hospice Interdisciplinary Group (IDG) as a team member with its responsibilities as outliners under IDG policy

    Able to work cooperatively, collaboratively and in a productive manner as part of a fully integrated interdisciplinary team

    Able to work in an interfaith environment

    Provide consultation/counseling to staff when requested

    Works with other clergy members in the community to facilitate their support of Hospice clients and families

    Provides direct service to hospice clients as their requests and needs dictate

    Documents visits and submits documentation in a timely manner per department Policies

    Assumes responsibility for promoting quality care, customer satisfaction and effective work processes

    Participates in Quality Improvement activities as assigned

    Consistently demonstrates understanding of own needs in regard to stress management, grief and loss resolution

    Understands and is able to describe the role of each team member

    Other duties as requested by supervisor

    II. Professional Accountability:

    Demonstrates a commitment to integrity in work habits and use of organizations resources

    Performs duties as assigned that may be outside his/her principle responsibilities

    Meets own learning needs. Participates in educational programs of the organization, including completion of all mandatory competencies, education or training

    Effectively communicates to transfer information, provide guidance and resolve conflicts

    Reports concerns and complaints regarding abuse, neglect and exploitation in accordance with Vermont State Law and the organization

    Enhance or improve organizations services by participating in team meetings, projects and committees

    Contributes to a positive work environment and responds in a constructive manner to change

    Documents the provision care and services in accordance to organizations policy
