Job summary
This is a Case Manager role where you will assist clients in gaining access to needed medical social educational and other services essential to meeting basic human needs. You will provide for client intake referral serve as an advocate for the client in obtaining supportive services for referral to other programs assessing needs of the client assist in developing billing for services providing program reports work as liaison between ASCEND and other agencies.- -Assisting clients in obtaining essential community resources social support skills networks that alleviate life stresses resulting from condition necessary enable maintain independent living.
- -Provide support education consultation directed exclusively well-being benefit client served assist maintaining independent living community.
