    Mobile Health Intake Coordinator – Includes Weekend Hours


    Duties of a Mobile Health Intake Coordinator may include the following but are not limited to:

    Primary Responsibilities:

    • Accompanies Mobile Unit to all screening events
    • Welcomes patients onto the Mobile Unit
    • Arrive patients in Centricity RIS and Cerner PM Office, obtain signatures for Hospital General Consent, scan photo ID, insurance care/CSP consent
    • Help patients fill out exam paperwork
    • Instructs patients with preparation for screening
    • Assures all supplies are on the Mobile for daily use
    • Schedules/dispatches Uber Health rides
    • Communicates with Patient Access for patient registration
    • Assures patient flow on the mobile unit
    • Communicate with community liaisons

    In Office Responsibilities:

    • Assist office staff with assigned tasks
    • Prepares files for upcoming screening events
    • Restock supplies on the mobile unit
    • Respond to phone calls and voice messages
    • Schedule appointments in RIS
    • Type event schedules
    • Make appointment reminder phone calls
    • Communicates with FQHA clinics
    • Prepare bags with consent books
    • Assist with and/or attend monthly educational tabling events

    Qualifications

    Required Qualifications: Associate's degree and at least 2 years of medical office and/or hospital clerical experience or, in lieu of degree, at least 4 years of medical office or hospital clerical experience. Knowledge of Medical Terminology.

    Preferred Qualifications: Bilingual in English and Spanish. Bachelor's degree. Experience in Radiology.

    • Successfully complete pre-employment physical examination and obtain medical clearance from Stony Brook Medicine's Employee Health Services*
    • Complete electronic reference check with a minimum of three (3) professional references.
    • Successfully complete a 4 panel drug screen*
    • Meet Regulatory Requirements for pre employment screenings.
    • Provide a copy of any required New York State license(s)/certificate(s).

    Failure to comply with any of the above requirements could result in a delayed start date and/or revocation of the employment offer.

    *The hiring department will be responsible for any fee incurred for examination.

    Anticipated Pay Range:

    The starting salary range (or hiring range) for this position has been established as $41,372 – $48,578 / year.

    The above salary range (or hiring range) represents SBUH's good faith and reasonable estimate of the range of possible compensation at the time of posting.

    In addition, all UUP positions have an additional $3,087 for location,
