Description
Washington County Mental Health ServicesCommunity Support Program Residential Programs
Community Support Division
Reports to: Program Coordinator
The WCMHS Community Support Program (CSP) - The Community Support Program (CSP) provides comprehensive community-based support services to persons with intensive and persistent mental health needs. Our services are individualized and emphasize health, wellness, and recovery.
General Residential Support offers a wide range of residential supports including licensed therapeutic homes, peer supported housing, and crisis housing. Our residential settings offer a trauma-informed, recovery-oriented approach. CSP operates several transitional housing programs designed to assist individuals with intensive mental health needs to find permanent affordable housing.
Single Steps: This Montpelier group home is an eight bed, licensed, Level III Community Care Home. Single Steps is a transitional living program with a targeted average length of stay of 12 to 24 months. The program supports individuals enrolled in Community Rehabilitation (CRT)/Community Support Program (CSP) through friendly daily interactions in a home like setting. Single Steps provides nursing oversight for medical issues. Residents receive additional support as needed from therapists, psychiatrists, and vocational staff. Resident's progress is reviewed monthly with progress measured through such means as reductions in hospitalizations and self-harm, reduction in substance abuse, maintaining stable relationships, and finding and maintaining employment. The facility is staffed twenty-four hours a day. Staff of this program are there to provide support, design activities, promote social interactions and personal development, and provide education and training in learning the skills necessary for independent living.
Segue House: Segue is a seven-bed licensed Therapeutic Care Residence and HUD funded residential facility located in downtown Montpelier. Segue currently offers transitional support (typically 6-18 months) to folks who may be dually diagnosed with severe and persistent mental illness and substance abuse disorders. The treatment offered is consumer-centered, individualized, and recovery-oriented. To be eligible, potential residents should present clear treatment goals related to their substance use patterns, practicing skills necessary for independent living, or seeking support around symptom management of their illness(es). Resident goals are reviewed in monthly treatment team meetings and supported by weekly group therapy meetings. Staff of this program assist residents in engaging in routines and behavior to achieve their team approved goals and living skills.
Road House: This home is a three-bed licensed, Level III Residential Care Home in Barre that houses individuals who require more long-term living arrangements in a 24/7 staff supported environment. Roadhouse provides nursing oversight for medical issues. Clients are chosen for this program if they have had previous difficulties living independently or in other group or residential settings. Staff of this program will aid with meal planning, vocational pursuits, medication, treatment and recovery planning, scheduling and transportation to appointments, and emotional supports.
Chrysalis House: Chrysalis is a 24/7 two-bed unlicensed Residential Home in Waterbury. Staff of this program will support individuals and develop social and daily living skills in a supported environment. Individuals are continually seeking to advance their autonomy and independence, through the pursuit of individually identified recovery goals. Assistance is provided with meal planning, vocational pursuits, medication management and treatment, in conjunction with recovery planning, scheduling and transportation to various appointments, and emotional supports.
Moody Court: Moody court is an independent residential apartment program adjacent to Chrysalis house in Waterbury. This program offers the benefits of a fully independent and autonomous residential environment for up to four (4) individuals at any given time. Staff of this program will assist in maintaining critical recovery supports, including medication management and administration, conflict resolution and ongoing social skill development 24/7. The program provides a unique opportunity to serve residents in the least restrictive environment possible, while maintaining needed supports within the community. This promotes autonomy and encourages continued recovery growth.
All programs will have schedules that include shifts in normal business hours, evening hours and asleep overnight hours.
