- Provide compassionate home-based care to patients with six months or less to live, focusing on enhancing their quality of life.
- Collaborate with an interdisciplinary team, including physicians, LPNs, CNAs, HHAs, social workers, chaplains, and volunteers to implement the patient's care plan.
- Conduct patient assessments, monitor vital signs, evaluate breathing, manage pain, and administer prescribed medications and treatments.
- Ensure accessibility to medical equipment, prescribed oxygen, medications, and other necessary supplies.
- Educate families on the patient's prognosis, changes in care plans, medications, and hospice goals.
- Adopt a holistic approach to care that nurtures the patient's mind, body, spirit, and family.
- Active, unrestricted RN license in the State of Michigan.
- At least one (1) year of relevant RN experience (required).
- Community health and a broad range of Medical-Surgical experience (preferred).
- Reliable transportation with a valid state driver's license and current car insurance.
