We are seeking a Part-Time Nurse Practitioner to join our Home-based Medical Care team. The successful candidate will see patients in Greene & Fayette Counties, PA for 10 hours per week.Responsibilities
- Establishes and maintains effective relationships with patients
- Performs an initial comprehensive assessment on all newly enrolled patients
- Develops a patient management care plan upon enrollment
