Job Description:

• The Audubon Clinic serves individuals with serious mental illness, the majority of whom are Hispanic and reside in the Washington Heights neighborhood of Upper Manhattan.

Requirements:

• Social Worker must be licensed
• Spanish language preferred

Role & Responsibilities:

• The social worker will carry a caseload of patients with serious mental illness and will contribute to our mission of providing recovery oriented services through case management and supportive therapies.
• The social worker will formulate multidisciplinary treatment plans, psycho-social assessments and progress notes.
• The social will be part of a multi-disciplinary team working with psychiatrists, nurses, peer specialists, occupational therapists and a team of other social workers.
