Siouxland Community Health Center has an opening for: HIV Case WorkerKnowledge of (socio-economic, culture and other) barriers to medical care and ability to diminish barriers on the patient's and SCHC's behalf.
Must have ability to handle confidential information on a need-to-know basis as defined by SCHC Successful candidate must be able to perform primary functions of position: Assist the Case Managers in working with the program patients.
Provide care coordination, linking case-management with access to care to include home visits. Facilitate patient advisory groups and education groups. Educate program clients in preventive health habits including harm reduction. Educate SCHC staff on HIV, HCV and STI programs, testing and patient advocacy. Educate community partners on HIV, HCV and STI programs, testing and advocacy. Provide HIV, HCV and STI counseling, testing, and linkage to care for assigned counties. Assist with social networking, program promotion and outreach for assigned counties. Assist with social media and marketing activities. Assess patients for social service, substance misuse and mental health needs and provide assistance and referrals. Enter data and track patient information and financials in Careware, REMI and SCHC chart. Coordinate support groups, schedules, locations and times.Utilizing first-hand knowledge of assigned tasks and responsibilities, demonstrate initiative in offering proactive suggestions for improvements in departmental workflow and processes.
Embrace and support SCHC's Process Improvement and Quality initiatives by participating in PI events and report outs and by supporting the PI and Quality teams in their initiatives.
