** Currently Fully Remote, but there is a possibility for some travel. At this time, hiring is open throughout IL. If they end up needing to travel, it will be in the region they live.

Case Manager RN JobHighlights

• Pay: $35.87 per hour, paid weekly
• Full-Time, Monday to Friday 8AM to 5PM
• Contract with potential to convert to permanent after 3 - 6 months

** Must be located and licensed in Illinois **

Job Responsibilities:

The Case Manager utilizes a collaborative process of assessment, planning, facilitation and advocacy for options and services to meet an individual's benefit plan and/or health needs through communication and available resources to promote optimal, cost-effective outcomes.

Qualifications

• Minimum 3-5 years clinical practical experience preferred
• Minimum 2-3 years Care Management, discharge planning and/or home health care coordination experience preferred
• Confidence working at home/independent thinker, using tools to collaborate and connect with teams virtually
• Must be located and licensed in Illinois
