The RN Start of Care Home Care Nurse is a key member of our Home Care team, responsible for initiating care for new patients in their homes.Responsibilities
- Conduct Start of Care Assessments: Perform thorough initial assessments of new patients in their homes, including physical, psychosocial, and environmental evaluations.
- Care Planning: Develop and implement personalized care plans, incorporating input from patients, families, and other healthcare professionals.
