- Conduct adult/geriatric assessments to include medical history diagnosis and treatment health education physician referrals case management referrals follow-up clear documentation according to Matrix guidelines protocols.
- Administer order point of care lab-based testing as needed.
- Work with physicians case managers social workers family members key caregivers ancillary medical personnel as appropriate.
