    A social services organization in California is currently seeking a new Service Coordinator for a promising opportunity with their growing team in Sonoma County.

    In this role, the Service Coordinator will be responsible for managing the delivery of services to regional center clients, including those with intensive physical or emotional needs and ensuring that the client's Individual Program Plan and its objectives are pursued.

    7am-9:30am to 5:30pm


    Patient Population:
    All ages (individuals that were diagnosed with an intellectual disorder prior to turning 18)

    Staff to Patient Ratio (caseload):

    1:75


    Responsibilities:

    The Service Coordinator will:
    Write concise, accurate and objective Interdisciplinary Notes regarding the current status of the client and related service coordination activities on a monthly basis
    Communicates effectively in person, on the telephone and in writing
    Read, interpret, and follow complex policies, procedures, laws, and regulations
    Interact tactfully and effectively with those contacted in the course of work
    Maintain current workload with accurate and complete documentation
    Conduct meetings, interviews and evaluations; interpret and document data
    Visit clients in their homes, schools, and day programs
    Record accurately and timely on time sheet and mileage forms
    Perform other duties, as assigned

    Qualifications:
    3+ years of experience in a field related to Developmental Disabilities with a Bachelor's Degree; or, a Master's Degree in a field related to Developmental Disabilities
    Knowledge of Human Growth and Development
    Knowledge of Developmental Disabilities and Case Management functions
    Solid assessment and documentation skills
    Patient oriented
    Great interpersonal skills
    Excellent communication skills (written and verbal)
    Strong attention to detail
    Highly organized

    Desired Skills:
    Bilingual
    Experience working with the IDD population
    Specific experience and/or education in one or more of the following areas: Social Work; Rehabilitation; Special Education; Child Development; *Clinical Counseling; Genetics; and, Developmental Psychology
