    Key Responsibilities for RN/LPN:

    • Conduct ongoing assessments of the patient's physical and psycho-social status
    • Collaborate with the Interdisciplinary Group (IDG), patient/family, and attending physician to develop a comprehensive care plan
    • Provide education and interventions for effective pain management and assess patient/family responses
    • Ensure availability of necessary equipment, supplies, and medications
    • Revise the IDG care plan as patient/family needs evolve
    • Document patient/family services accurately and promptly
    • Participate in regular IDG meetings

    What We're Looking For:

    • Active, unrestricted RN/LPN license in the State of Michigan
    • Minimum of one (1) year of relevant experience as an RN or LPN (required)
    • Preferred: One (1) year of management experience
    • Preferred: One (1) year of home health experience
    • Current CPR Certification
    • Preferred: Community health and a broad range of Medical-Surgical experience
    • Reliable transportation with a valid state driver's license and current car insurance
