    Summary:
    Provides diabetes education for Patients in Family Practice setting to help develop educational material, coordinates visits, and maintains records.

    Educates patients with diabetes on the pathophysiology of diabetes, the psychological issues of having the disease, oral and injectable diabetic medications, and sick day management.

    Teaches patients how to monitor blood sugars and manage blood sugars with food and exercise. Actively participates in patient care and collaborates with staff.

    Essential Responsibilities:
    Responsibilities include proficiency in documenting medical visits, procedures and placing orders. Completing charts accurately for physician review, and patient education.
    Documenting history of present illness, past medical history, review of systems, and assessment plans.
    Entering medications, ordering lab orders (bloodwork/pathology) and setting reminders for follow-up appointments.


    License/Certification:
    Registered Dietitian - RequiredQualifications:Bilingual Spanish - RequiredFamily Medicine experience preferred.
    EMR experience, and knowledge of ICD-10 codes preferred. Ability to work in a fast-paced environment, able to adapt to schedule changes.
    Excellent communication and interpersonal skills are required. Must be able to type 50+ WPM and have basic computer knowledge. Previous scribing/ medical terminology experience is a plus.
