    Description
    SUMMARY

    Responsible forproviding direct patient care under the supervision of physician practitionersin specifically-assigned centersand within the patient care on-call rotation of the primary care organization.

    ESSENTIAL DUTIES AND RESPONSIBILITES


    • Medically manages a defined caseload of geriatricpatients in skilled nursing centers, assisted living or personal care centers,home health, hospice, or community settings.
    • Makes periodic, pertinent patient visits to thecenter or other site of care.
    • Provides appropriate, timely medical orders.
    • Provides appropriate, timely and pertinentdocumentation and assessments.
    • Communicates with patients and/or responsibleparties appropriately and in a timely manner.
    • Committed to service to the patient, the community,and the mission and vision of Physician Services.
    • Manages medical staff relationships with nursing,patients, and families.
    • Develops a close and collegial relationship with allphysicians and non-physician practitioners.
    • Participates in physician led, evidence based, high quality, patient centered care initiatives, quality measures, and/or clinical practice guidelines.
    • Maintains effective inter-departmental andinter-organizational communication to provide positive working relationships.
    • Complies with established budget restraints.
    • Maintains appropriate supplies and equipmentnecessary to perform job and in accordance with established departmentalbudget.
    • Supports new information technology systems with anemphasis on electronic medical record system compatible with organizationsystems.
    • Participates in organization surveys and anysubsequently required reports.
    • Attends and participates in continuing educationalprograms to keep abreast of changes in professional field as well as tomaintain current license/certification asrequired.
    • Attends and participates in mandatory in-services.
    • Follows established safety procedures whenperforming job tasks and/or working with equipment.
    • Honors patients' rights to fair and equitabletreatment, self-determination individuality, privacy, property and civilrights.
    • Complies with the Corporate Compliance Program.
    • Reports job-related functions /tasks that involveoccupational hazards including exposure to blood and body fluids and others asnecessary.
    • Follows established safety regulations to includefire protection/prevention, smoking regulations, infection control, etc.
    • Complies with all Privacy and Security, AdverseEvent Response, and Disaster Security and Business Recovery programs.
    • Promotes the image and reputation of the System byexhibiting servant leadership and providing direct and open lines ofcommunication.
    • Contributes to the work of committees, workgroups,project management, and other collaborative efforts of the System.
    • Performs other duties as necessary to ensure thesuccess of the System. SKILLS AND ABILITIES
    • Knowledge and compliance with rules and regulationsgoverning medical care and payment for medical services.
    • Knowledge and compliance with the proceduralguidelines for the organization and its client centers. MINIMUM QUALIFICATIONS
    • Licensed as a Registered Professional Nurse meetingthe Georgia Board of Nursing requirements to engage in advanced nursingpractice and;
    • Holds a Master's degree or other graduate degreeapproved by the Board of Nursing.
