    Description


    NANI is seeking a Patient Coordinator to perform patient outreach and ensure appropriate follow up and transitions of care.

    Summary: The Patient Coordinator would be responsible for the multiple aspects of care coordination related to the CKCC Value Based Care Program.

    Essential Functions:
    • Performs patient outreach/communications for care coordination.
    • Track and outreach to complex and high utilizer patients.
    • Assists in Transitional Care Management follow up and patient scheduling.
    • Coordinates scheduling for new incoming referrals
    • Helping with the post-transplant process of patients returning to their nephrologist by coordinating appointments and closing the loop on referrals back to nephrologists
    • Collaborates with all levels of the clinical team to improve processes, communication, and team skills needed to provide the best care to our patients.
    • Utilize clinical judgement, independent analysis, critical thinking skills, time management skills and detailed knowledge of care coordination.
    • Actively participates in committees/meetings as needed.
    Minimum Qualifications/Education:
    • Medical Assistant or an equivalent combination of experience
    • 2-5 years' previous experience in healthcare, preferably nephrology
    Preferred Qualifications:
    • Fluent in Spanish both written and oral
    • Bachelors degree or higher in a healthcare related field
    • Strong computer skills and proficiency in MS Office and Outlook, especially Excel
    • Excels at developing strong patient and family relationships that fosters engagement and best outcomes
    • Minimum of 3 years' experience in renal, ER, ICU, or Ambulatory Care settings
    • Demonstrated knowledge and understanding of managing patient satisfaction outcomes
    • Proficiency with documenting and navigating electronic medical records (EMR), especially Acumen Epic
