    GENERAL PURPOSE OF JOB:

    This position provides technical and clerical support to the Social Work and Case Management departments. This position is responsible for the support of functions necessary to accomplishing the objective of the department. These include but are not limited to telephone support, patient support, clinical support, patient discharge and follow up.

    This role will assist patients and families with setting up and securing meals, lodging and transportation as well as referring families to appropriate resources available to them.

    The care coordinator technician operates within the framework of the Driscoll Children's Hospital mission, vision, and values.

    Knowledge:

    • Medical terminology preferred.preferred. Prior medical office or hospital experience preferred Ability to understand complex situations and interpersonal dynamics to effectively handle escalated customer and co-worker needs.

    Skills:

    • Requires a well-organized individual with an excellent capacity for effective time management.
    • Demonstrates ability to establish and maintain effective working relationships with the provider office staff and peers. Demonstrates ability to operate personal computer programs.

    Basic Microsoft Office Excellent communication skills Responsibilities:

    • Coordinates completion of and transmits information to post-acute provider i.e., home care, nursing home, other discharge/transfer destinations and services as directed through the hospital software system or other processes identified. Assists Case Manager RN and Social Worker with transactional support (faxing, follow up calls, etc.)
    to arrange for discharge of patients with licensed healthcare oversight:


    • Durable medical equipment (DME)
    • Home Health, Palliative Care, Hospice Transportation, meals and lodging
    • Authorizations Contacts agencies regarding bed availability and accessibility
    • Develops strong working relationships with various community agencies to promote timely patient discharge and positive financial outcomes.
    • Facilitates the ordering of Durable Medical Equipment as directed by the Case Manager Nurse /Social Worker. Observes all compliance requirements under CMS guidelines and IMPACT Act requirements, related to Patient Choice and documentation. Support Case Managers and Social Workers with any delegated tasks Communicate with patient/caregivers as needed/directed and escalate any concerns to the CM/SW immediately Proactively assist in obtaining medical authorizations for facility placement in conjunction with the facility.
    EDUCATION AND/OR EXPERIENCE:


    • Must have a high school diploma or equivalent; college education preferred.
    • Experience or knowledge of Social Security Administration SSI/SSDI programs.
    • Medical Terminology preferred.
    • Prior medical office or hospital experience preferred.
    • Basic Microsoft Office skills required.
    • Experience with Texas Health and Human Services Commission (HHSC) or the Department of Aging and Disability Services (DADS) a plus.
