• Limited patient volume of highly complex patients 55 yrs.

• Probably see 6 10 patients per day
• Provide comprehensive care to vulnerable and comorbid patients of the highest complexity.
• All patients seen in a clinical setting with transportation provided if necessary
• Team of physician and APP cover a patient panel of approximately 150
