    We are looking for a Medical Social Worker to join our team for Home Visits in Hospice care. Candidates must be licensed Social Workers with at least one year of experience in a healthcare setting.

    Responsibilities

    The Medical Social Worker in hospice plays a crucial role in providing social services under the direction of a physician and Interdisciplinary Group. Their main tasks include:

    • Helping patients and their families understand social and emotional factors impacting health
    • Contributing to the development of comprehensive care plans daily
    • Educating the healthcare team on available community resources
    • Supervising bereavement services if necessary, ensuring the family's needs are met

    Qualifications

    License Requirements

    • Bachelor's Degree in social work or related field
    • Minimum of one year of social work experience in healthcare
    • Current CPR certification
    • Valid Driver's License and access to reliable transportation

    Additional State Specific Requirements

    • WV: Social Worker license from the State Board of Social Work
