    Description


    The Quality Assurance Sepsis Coordinator is a key member of our Sepsis Care program functioning as a clinical expert and team leader.

    The position is responsible for evaluating sepsis care within the organization, identifying opportunities for improvement, and providing real-time and formal education for all disciplines involved regarding sepsis care, core measures, readmissions and other related clinical quality measures.

    The Quality Assurance Sepsis Coordinator works under the supervision of the Quality Management Director/Chief Quality Officer and will also support the leader in all process improvement projects for valuebased purchasing programs.


    Required:

    Graduate of an accredited School of Professional NursingPreferred: Bachelor's Degree in NursingExperience: Minimum of two years of nursing experience in an acute care setting.

    Experience working with clinical data sets, data management, analysis, and processimprovement initiatives preferred.


    Knowledge, Skills & Abilities:
    Knowledge of basic statistics and proficiency in the use of MedHost (facility's EMR) electronic medical record are preferred. Familiarity with Microsoft Office or Google Suite programs is required. Excellent written and verbal communication skills are required. Ability to interact with diverse physicians, clinical leaders, and other staff members.


    Required:
    Current licensure in professional nursing from the New Mexico State Board of Nursing or compact RN license


    Required:
    Current BLS (Basic Life Support) - Due by close of business on the last day of the month after one (1) full month of work


    Required:
    Current ACLS (Basic Life Support) - Due by close of business on the last day of the month after three (3) full months of workPrimary Duties and Responsibilities:

    • Collaborates with the clinical and ancillary departments to enhance the overall success of the sepsis and value based purchasing programs.
    • Serves as the subject matter expert (SME)/liaison for the state's value based purchasing program, to include planning, leading and coordinating process improvement initiatives/projects that will lead to improved performance.
    • Continually evaluate patient and program progress towards goals and apply appropriate interventions to ensure effective outcomes.
    • Develops reports and presentations on sepsis care processes and VBP related outcomes data with weekly, monthly, quarterly, and annual trends to present to all keystakeholders.
    • Works collaboratively with the medical and hospital staff to provide a multidisciplinary approach to the care of the sepsis patient.
    • Responsible for developing patient teaching materials and staff development regarding care of the Sepsis patient.
    • Collaborates with the Quality Management and Clinical Documentation Specialist teams to improve provider documentation related to Sepsis criteria, and core measureabstractions.
    • Able to train and educate, give technical instruction, provide leadership, and generally raise the level of performance of others.
    • Participates in regulatory and accreditation reviews under the supervision of the Quality Management Director.
    • Serves as the co-chair of the Sepsis Program Committee.
    • Performs other duties and responsibilities as assigned.
