Job summary
The Clinical Care Assistant provides direct patient care in the home setting for individuals recently discharged from skilled nursing facilities. This role supports continuity of care by delivering wound care, care management support, and other approved medical services under the direction of a licensed Medical Director.Responsibilities
- Provide in-home clinical services to patients following discharge from skilled nursing facilities
- Perform wound care, dressing changes, and monitor healing in accordance with established care plans and protocols
- Conduct patient assessments including vital signs, symptom monitoring, and medication reconciliation
