    Lighthouse Behavioral Health Solutions (LBHS)providesindividual, outpatient, intensive outpatient, partial hospitalization, residential, psychiatric services, and medicationassistedtreatment for mental health and substance abuse treatment needs.

    Our comprehensive approach to addictions treatment provides a continuum of care that begins with asking for help and continues through to ongoing peer support.

    In addition to ourstate-of-the-arttreatment facilities and peer support centers across Central Ohio, we work to remove barriers and promote access to care through our integrative approach and evidence-based clinical treatment.


    Position:

    Care CoordinatorJob Summary:

    This position provides case management services to adults withidentifiedmental health or substance use diagnosisDuties and Responsibilities:Duties include, but are not limited to:Provideongoing case management services to residents, including regular check-ins, goal setting, and progress monitoring.

    Serve as a liaison between the recovery housing program and treatment providers,facilitatingcommunication and information sharingregardingcase management services.
    Coordinate and schedule appointments with treatment providers, ensuring residents have access to necessary services and follow-up care.
    Assistresidents in accessing community resources and support services, such as employmentassistance, educational programs, and legal services.
    Advocate for residents' needs and rights, ensuring they receiveappropriateand quality care.
    Conduct regular meetings with residents to addresscommon challenges,provideeducation, and promote community-building.
    Collaborate with the recovery housing program staff to ensure a safe and supportive living environment for residents.
    Maintainaccurateand up-to-date documentation of resident interactions, progress, and treatment plans.
    Participate in team meetings and treatment team meetings to discuss resident progress and develop strategies for improved care.
    Providecrisis intervention and support to residents in times of relapse or other emergencies.
    Conduct urine screens and medication counts at the request of clinical staff.
    Coordinate discharge planning and support residents in transitioning to independent living or otherappropriate levelsof care.
    Stay updated on current trends, research, and best practices in the field of addiction recovery and mental health.
    Collaborate with external agencies, community organizations, and treatment providers to enhance the resources available to residents.
    Attend training sessions, workshops, and conferences to expand knowledge and skills relevant to the role.
    Uphold ethical standards and confidentiality guidelines in all interactions and documentation.
    Contribute to program evaluation and quality improvement efforts,providingfeedback and suggestions for enhancement.
    Foster a culture of respect, empathy, and support within the recovery housing program.


    Required Experience/Abilities:
    Possesses superb written and spoken communication skillsExcellent interpersonal skills with clients, colleagues, and community resourcesExcellent time management skills; organized and able to prioritizeComfortable in a fast-paced environment with multiple clientsAble to organize and managelarge amountsof files, schedules, dates, and informationCalm and cool-headed with a positive attitudeSelf-directed and self-motivatedComfortable with building personal relationships and dedicated to helping othersValid State of Ohio Driver's License and insuranceMust pass BCI check, all Corporate Compliance checks, and employment drug screenDesired Certifications and Experience:Required :CDCA or LCDCIIState of Ohio Driver's LicensePreferred :LSWBachelors Degree in a behavioral health field (Social Work, Psychology, Counseling, etc.)
