    If you are mission driven and want to help the lives of people in your community, we have a place for you on our Team The Engagement Specialist serves as the central wellness point of contact for clients in the New Vista healthcare system.

    This position ensures coordination of care team members and provides linkages to health and wellness services.

    Serving as the client's advocate and working to overcome obstacles to care, this position ensures the client's voice is central.

    This position requires a general knowledge of the healthcare system and a proactive approach to learning about ever-expanding services within healthcare, benefit assistance and basic health screening processes.

    The Engagement Specialist thinks outside the box and finds ways to connect clients to the right care, ensuring a critical "no wrong door" approach - that any client is provided a universal gateway to enter any New Vista service.

    The Engagement Specialist is a positive team player that promotes a culture of inclusion and participation.

    Required Education and ExperienceHigh School graduate or equivalentExperience/education in healthcare and/or customer serviceValid driver's license, proof of auto insurance and reliable transportationPreferred Education and ExperienceExperience providing case management servicesCMA or bachelor's degree in Psychology, Social Work, Public Health or related fieldExperience in integrated care service (e.g., both behavioral and physical healthcare)Essential FunctionsReasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

    Ensure clients receive services (virtual or in-person) in a timely and effective manner identifying and removing barriers to accessFacilitate communication between clients, family members and the care team to ensure client satisfaction and quality of careEvaluate social determinants of health and link clients to resources such as housing supports, food, basic needs, financial assistance, and employment resources Intervene when appointments are missed to determine reason, barriers to access and identify any desires for care to be adjustedIdentify and links clients to proper services that address those who have limited English proficiency, are deaf or hard of hearing, or who require age-related or cognitive accommodationsEnsure continuity of care between internal and external providers, support transitions or adjustments to care and provide linkages to community resourcesAdvocate with providers and payors for proper healthcareWith a whole health lens, determine creative solutions to barriers to care directly or through facilitating connections to internal or external resources for issues such as transportation, housing and wellnessBecome familiar with the network of medical providers in the area and how to apply and access benefitsTrack appointments, medication, and other wellness needs for clients on caseload as neededConnect with the care team and help ensure clients do not fall through the cracks using creativity, teamwork and a whole health approachCross train with other aspects of the Call Center, including Helpline and National Suicide Prevention Lifeline/988While we welcome all to apply, there are some position specific background stipulations that must be met to comply with various regulatory and contractual requirements.
