- Experience in Case Management and/or homecare is advantageous.
- Proficiency in Bilingual Spanish is desirable.
- Licensed as a Registered Nurse in the State of SD and/or meeting multi-state licensure requirements is obligatory.
- Licensure as a Registered Nurse in the State of Minnesota is mandatory. Must be obtained within 6 months of job placement.

Avera is currently seeking Registered Nurses (RN) Case Manager interested in providing exceptional nursing care in Avera's patient-centric environments.

Position Highlights:
Job Summary
The nurse clinical case manager aids in patient care management within the clinic setting. The primary role involves overseeing care for the primary care practice's patients to enhance education, self-management support, and prompt healthcare provision. Tasks include establishing and overseeing care management procedures and assisting primary clinical teams in these endeavors. Identifying the high acuity patient group and ensuring care coordination for them is also crucial. The role includes some patient triage. The nurse clinical case manager collaborates with clinic leadership and the medical director (lead physician) to customize this role for the patient panel's needs and the primary care teams. This individual works closely with the AMG Director of Patient Care Coordination/Medical Home and others to aid in developing, maintaining, and reporting quality measures within the medical home model. The registered nurse will demonstrate performance aligned with professional practice standards, care, and the Nurse Practice Act.
