About Us

We help women get more life out of life, together. We have built a new, collaborative model of value-based primary care for women 50+ to solve long-standing problems: rushed appointments, long wait times and care that's generalized rather than specialized to women's preferences and concerns.

The Primary Care Physician's Role:

The primary care physician is a key member of our interdisciplinary care team. They evaluate our patients' acute and chronic conditions manage their multiple medications mental health socio-emotional needs and provide empathetic patient education. The pcps work closely with fellow clinical teammates to provide comprehensive primary care while direct patient care is the primary focus of the position they also assist in development implementation and achievement of key organizational objectives metrics functionally managing population health according IHI quintuple aim adopting best practices coding compliance achieving HEDIS quality metrics we are also on hunt future talent residents fellows all levels encouraged apply h4 core qualifications:

Licensed md/do board certification family medicine abfm internal medicine abim
Experience complex diseased longitudinally over time diabetes cardiovascular disease menopause thyroid disease osteoporosis weight management etc
Understanding willingness learn practice value based car including cost quality manage well devising individualized plans educating counseling health maintenance prevention geriatric particularly experienced polypharmacy appropriate medication reduction physical gynecological examinations elderly experience working clinical situations adaptability problem solving critical thinking up knowledge advancements technologies alignment herself mission dedication high-quality patient live best lives culture collaboration excellence sleeves roll grows clinicians resonate values her first curious always learning own act unstoppable.
