    Position: HIV (Non-Medical) Case Manager

    Supervision: Reports to Program Manager

    Status: Full-Time (Minimum 37.5 hours/week)

    Classification: Salaried (Exempt)

    Pay Rate/Range: Minimum Base Salary $50,000/year

    Benefits Offered: After employed for 90 consecutive calendar days, eligible for paid Sick Time, as well as participation in the Group Health, Dental, and Vision and 403(b) Plans (with a Company match on the 403(b) Plan contribution after 24 months). After employed for six (6) months, eligible for paid Vacation Time.

    POSITION PURPOSE:

    The Non-Medical Case Manager is a Full-Time position requiring a minimum 37.5 hour work week. This position works closely with a broad patient demographic, including those living with HIV/AIDS and Hepatitis C, and who have multiple psychosocial and/or health related needs. Services are based upon a strength-based approach that includes treatment adherence counseling, coordination and follow-up of medical treatments, client advocacy, and assistance in obtaining housing, financial support, social support, and any other needed service. The goals for this position are to (1) ensure that clients with HIV/AIDS have timely access to comprehensive medical care and social services; and (2) prevent disease transmission and delay of HIV progression; and (3) promote and support client independence and self-sufficiency.

    ESSENTIAL FUNCTIONS

    Key Performance Area: Screening

    1. Works in collaboration with the Program Manager and Clinical Staff to identify current and new clients who can benefit from Case Management services.

    3. Works with agencies in the HIV/AIDS service system to engage individuals infected with HIV, increasing the number of clients enrolled in care at Harmony Healthcare (HHO).

    4. Responds within 24 hours to requests for Case Management services.

    5. Schedules a screening time within 5 days of the initial request.

    6. Completes the screening to determine eligibility for Case Management services.

    7. Provides backup to HHO's HIV Testing program.

    Key Performance Area: Orientation and Assessment

    ● Provides an overview of Case Management services, including the role and responsibility of the Case Manager.

    ● Completes a comprehensive assessment within 15-30 days of initial screening to determine the client's strengths, resources, needs, and concerns.

    ● Works with clients to develop an individual Action Plan which includes realistic, measurable, and mutually acceptable goals that are based on the results of the assessment.

    ● Identify action steps needed to achieve each goal, including target dates for accomplishment of the stated goals.

    ● Identify referrals made to other providers/services in connection with action steps.

    ● Copy plan and give to client and file original in client's file.

    Key Performance Area: Referrals and Case Coordination

    ● Attends and participates in HIV Collaborative Meetings, as scheduled.

    ● Enrolls Case Management clients in HHO's Extended Support Program.

    ● Identifies and contacts community-based organizations, primary care providers, housing services and other needed providers to establish referral agreements and to coordinate collaborative efforts with clients.

    ● Completes all documentation to show evidence of the time, date, place, and description of each Case Management service.

    ● Documents progress made toward goals by writing notes in the Case Management online database.

    ● Ensures client continues to meet eligibility requirements.

    Key Performance Area: Continued Contact

    ● Face-to-face interaction with client at least every 90 days.

    ● Completes review of Action Plan every 90 days; making any changes, additions or deletions to current services.

    ● Obtains client's signature on Action Plan review indicating the agreement for continued contact and case management services.

    ● Develops an intervention plan to re-engage client, if face-to-face contact is not maintained and includes the intervention in the Action Plan.

    ● Documents treatment adherence activities in client's file; these include keeping medical appointments, taking prescribed medication, refilling prescriptions, etc.

    ● Completes reassessment annually including the development of a new Action Plan.

    Key Performance Area: General

    ● Protects patients' rights by maintaining confidentiality of medical, personal, and financial information by following HIPAA guidelines.

    ● Promotes and practices HHO's mission and values, and follows its policies, processes, and procedures.

    ● Works effectively and professionally with a diverse population treating all clinical staff and patients with the utmost respect at all times.

    ● Participates in all required training and certifications including, but not limited to: HIPAA, Biohazardous Waste, CPR/BLS, 500/501 Rapid HIV testing, and Rapid HCV testing.

    ● Performs rapid HIV and HCV testing (when needed) and helps to ensure all documentation is scanned and stored according to Florida Department of Health guidelines.

    ● Attends and assists patients and clinical staff during community outreach testing events. Also may be required to attend other community outreach events such as GayDayS, Pride Events, etc. Advance notification of dates/times will be given to allow for proper scheduling. On these dates, work times may be modified to ensure employees do not exceed budgeted hours.

    CORE DUTIES AND RESPONSIBILITIES

    ● Face-to-Face Patient Case Management: Intake Screening verbal/written, Orientation, Assessments, Regular Updates to Action Plan.

    ● Referrals and Care Coordination.

    ● Continued Contact and Source Outreach.

    ● File/ Record Management.

    ● Perform Rapid HIV and HCV testing, as needed, after completion of training program.

    ● Regular, dependable attendance and consistent satisfactory or above job performance.

    ● Other duties, as assigned.

    EXPERIENCE AND MINIMUM QUALIFICATIONS (Preferred unless indicated as a Requirement)

    ● Vaccinations and immunizations recommended for healthcare workers by the Florida Department of Health, including; COVID-19 vaccination and Mpox vaccination. (May be Required)

    ● English oral and written proficiency. (Required)

    ● Proficiency with Microsoft Office products (Word, Excel, PowerPoint, Teams, etc.). (Required)

    ● Proficiency of other languages welcomed (particularly Spanish and Creole).

    ● Word Processing.

    ● Experience with Apple products and apps.

    ● Ability and willingness to quickly learn new programs and systems.

    CERTIFICATIONS, LICENSES AND REGISTRATIONS (Preferred unless indicated as a Requirement)

    ● Bachelor's Degree in a Human Services or Healthcare-related field of study. (Required)

    ● Prior working experience with the HIV/AIDS community.

    ● Valid FL Driver's License and access to reliable transportation (with current vehicle registration and insurance). (Required)

    ● Use of public transportation to events and other locations may be an option as long as work schedule requirements are met.

    PHYSICAL DEMANDS

    The physical requirements described below are representative of those that must be met to successfully perform the essential functions of this position. Reasonable accommodation may be made to enable individuals with a qualifying disability to perform the essential functions.

    ● This position is generally performed in a mobile/field office environment; 55% sitting, 30% standing or driving, and 15% walking.

    ● This position routinely utilizes standard office equipment such as laptops, computers, phones, photocopiers, and filing cabinets.

    ● Ability to lift/move 25 pounds (or more).

    ● Occasional travel to participate in community outreach events may be required, including GayDayS, Pride Events, community testing events, etc.

    ● Travel may also be required to client homes, when needed.
