    Job Description

    Overview:

    We are seeking a dedicated professional to provide direct support to individuals served, including assistance in personal care activities. The ideal candidate will also perform required training and documentation necessary for group and individual participant goal achievement.

    Responsibilities:

    • Act as Case Manager for individuals, maintaining case records and working with participants and their teams to develop and follow-up on Individual Support Plans (ISPs).
    • Conduct follow-up to ensure delivery of ISP requirements, including direct support.
    • In the absence of managerial staff, act as site contact person, opening and securing the building.
    • Develop and implement behavior management interventions for individuals, in conjunction with the clinical team.
    • Work with individuals' needs related to home, work, and community environments, meeting with parents, guardians, providers, and other agency personnel.
    • Provide individual and group programming, focusing on age-appropriate behaviors.
    • Evaluate individuals' needs in areas like daily living, meal preparation, personal care, occupational skills, and more.
    • Foster growth by teaching opportunities for choice, initiation, and risk-taking.
    • Treat individuals with respect and dignity, adapting to age and cultural differences.
    • Participate in Interdisciplinary Team activities, as assigned.
    • Act as a role model and provide contextual training for staff in the integrated therapeutic model.
    • Maintain written information on instructional units, materials used, and approaches taken.
    • Assist in screening prospective participants and supervising staff effectiveness through performance evaluations.
    • Transport participants, as needed.
    • Communicate program/personnel issues or questions to the Program Manager.
    • Provide assistance with personal care needs, including dining, toileting, and transfers.

    Qualifications:

    A Bachelor's degree in Education or a related field, along with one year of experience in human services. Alternative qualifications: an Associate's degree or High School diploma, with two years of experience working in a program for people with mental retardation or developmental disabilities. A valid driver's license is required.

    Estimated Salary: $50,000 - $70,000 per year, depending on experience and location.
