    Bon Secours Program for Diabetes Health is hiring a CNS at their Memorial Regional Medical Center location. The Clinical Nurse Specialist will be responsible for implementing a patient-centered and outcomes-driven program of care for hospitalized individuals living with diabetes. Position responsibilities include implementing state-of-the-science diabetes care and diabetes self-management training (DSMT) using evidence-based practice protocols, guidelines and clinical nursing judgment; developing the clinical expertise of the healthcare team; and leading and facilitating system-wide change for the hospitalized population.

    Essential Job Functions:
    Deliver clinically expert diabetes care & patient self-management training

    Conduct an advanced nursing assessment, formulate nursing diagnoses, and develop an individualized plan of care using evidence-based practice guidelines, protocols and clinical nursing judgment

    Implement national clinical practice standards of diabetes care and DSMT and document care in the electronic health record

    Collaborate with interprofessional team members, and participate in meetings and rounds

    Provide and/or participate in educational programs for interprofessional staff to promote uniform understanding and implementation of evidence-based diabetes care

    Mentor professional nurses and other healthcare team members in acquiring the knowledge, skill and confidence to deliver evidenced-based diabetes care

    Collaboratively develop system-wide diabetes program

    Strategize development and implementation of diabetes program strategies, including but not limited to policy, procedure and protocol development, to coordinate diabetes care across the continuum.

    Communicate regularly with diabetes program director(s) verbally, through written report and by participation in scheduled diabetes program meetings

    Ensure care delivered to each patient is in accordance with the Joint Commission, National Standards for Diabetes Self Management and the AACE and the American Diabetes Association as appropriate

    Comply with federal requirements set forth under the Centers for Medicare and Medicaid Services Conditions of Participation

    Maintain accurate record-keeping to meet productivity standards and practice effective time management and prioritizing of tasks
