    How You Will Make a Difference

    As we expand into new markets, we are seeking a Physical Therapist who aligns with our core values of Trustworthiness, Capability, Compassion, Proactivity, and a strong Calling to serve patients in their homes. The Physical Therapist tackles each day with a fervent dedication to caring for patients individually, aiding them in regaining independence and achieving health milestones. In this role, you'll establish the standard for the patient experience at VitalCaring. Your devotion to your calling is recognized through the meaningful relationships you forge with patients and the flexible schedule enabling work-life harmony.

    As the Physical Therapist, you will:

    • Evaluate and assess patients to determine care suitability
    • Utilize tools and resources to create patient-specific care plans
    • Communicate with the care team, physicians, payors, patients, and families
    • Deliver high-quality skilled care and oversee other team members
    • Document care daily in our EMR system
    • Foster a caring culture through individual responsibility and teamwork

    • Passionate about learning and inspiring others
    • Enthusiastic about working at the highest PT license level
    • Proficient with technology, HCHB experience is beneficial
