Job summary
This position assesses patient physical psychological social spiritual educational developmental cultural and discharge planning needs reviews patient history and physical with patient family assures completion within appropriate timeframe Reviews available information obtained by other health care team members Reviews diagnostics laboratory data reports abnormal results to physician appropriate caregivers Completes assessment reassessments according to patient need outlined in policy Formulates plan of care including discharge plan utilizing assessment data Patient family health team input Initiates plan of care based on patientspecific needs Assessment data medical plan of care Goals for patients are realistic measurable developed conjunction with thepatientfamily Considers physical cultural psychosocial spiritual age specific educational needs of thepatientintheplanofcare Planscareincollaborationwithmembersofthemultidisciplinaryteam Reviewsandrevisestheplanofcare reflectchangingpatientsneedsbasedonevaluationofthepatientsstatusQualifications
- Must possess a valid current or temporary Alaska LPN license.
- Requires knowledge skills normally demonstrated by an Associates degree in nursing.
- BLS certification is required May require ACLS PALS additional certification continuing education area practice
