Job summary
The Discharge Planning Specialist is a member of a collaborative person-centered care team that provides enhanced coordination with inpatient hospitals and emergency departments.This position is responsible for solidify services, supports and documentation needed to facilitate hospital discharge and rapid release from EDs and serves as the single point of contact for discharge coordination with the RBHA and involved stakeholders.
The Discharge Planning Specialist will implement the Copa Hospital Readmission and Discharge Coordination protocol under the oversight of the Copa Clinical Director and Director of Medical Management.
They will collaborate with the Inpatient Team, RBHA, outpatient team-centered team, family or natural supports to develop an individualized plan promoting long term health reduction likelihood re-admission.
- Understand medical orders post-discharge/post-visit.
- Schedule post-discharge appointments within 72 hours.
- Ensure follow through on recommendations services identify gaps service referrals track coordinate identified referrals discharge plan travel attend staffing appointments obtain vital records community-based appointments answer question provide support after member discharged CPRP notifications hospital admissions/discharges alerts health information network available answer any questions provide support necessary after member discharged maintain company productivity quality measures complete required documentation timely manner consistent guidelines manage complete assigned work tasks deadlines participate regular staff meetings training programs clinical supervision may be part on-call rotation after hours other duties assigned
