Description

AdvisaCare, a leading provider of home health care services, is currently seeking a Full Time or PRN Home Health Physical Therapist to join our team As a Physical Therapist with AdvisaCare, you will provide quality care in patients' homes, helping them to regain independence and improve their quality of life.

Responsibilities

Conduct initial and ongoing patient assessments to determine individualized treatment plans and goals.

Develop and implement physical therapy interventions based on best practice guidelines, patient needs, and physician orders.

Monitor and document patient progress and modify treatment plans as needed.

Educate patients and caregivers on exercises and activities to improve mobility, strength, balance, and coordination.

Collaborate with interdisciplinary team members to ensure delivery of quality, holistic care.

Adhere to all legal, ethical, and regulatory guidelines and standards.

Requirements

Valid and current Physical Therapist license in the state of Michigan.

Minimum of one year of experience as a Physical Therapist, including home health experience, is preferred.

Strong clinical skills, including assessment, treatment planning, and implementation.

Ability to work independently and as part of a team, with excellent communication and collaboration skills.

Flexible and adaptable, with a positive attitude and customer-focused approach.

Ability to navigate technology and electronic medical records.
